My daughter right now age is 24 . From her childhood days she has problem in understanding basic facts ...she was not good in studies and perhaps she realized that she was not going well in studies cornered herself , as a result no friendship was developed with anybody. IQ test was done in Nair hospital in Mumbai and found a touch of autistic but not severe and doctors said she has to build up social skill to improve herself . with help of writers she passed out ssc from JK School thane followed by HSC & bca As she not fit for higher studies and lacks interest for higher studies , we did not force her to do higher studies . Recent improvement is noticed in communication skill but lacks maturity in terms of her age ( vis a vis today's G age group) She perhaps sings more a less well and we admitted her in Suresh wadkar's Ajivadsn musical academy for sastriya sangeet programe in thane branch and perhaps with songs her language is developed a bit . Communication in eng , Hindi & Bengali as such ok but lacks speaking skills .. But , we think , if she is joined in a group where skill development takes place , where she can find girls of her category in which she may find a different skill suits her interest ( which we are not aware ) . We noticed her understanding skill & expression of thoughts are better but at 24 there is some obstacles Since , she is our only daughter , we are concerned and seek advise to meet right person / organisations for proper guidance for welfare of daughter as ,we think, if enrolled in a particular course / put into activities for skill development programmes ( of her interest) her self esteem factors can increase and better mould is possible Kindly understand n guide Thanks
Ans: It sounds like you’ve made thoughtful and supportive choices for your daughter’s growth, especially through music, which is helping her communication skills blossom. To further nurture her social skills, self-esteem, and interests, several steps might be particularly beneficial. Connecting with nearby NGOs and parent support associations could be a valuable starting point, as many offer structured programs that focus on building social skills, independent living skills, and even employment readiness for young adults with autism. These organizations can provide both community support and access to programs specifically tailored for people with similar abilities, allowing her to meet others and gain confidence in a comfortable setting.

Skill development centers in Mumbai and Thane, such as the **Ummeed Child Development Center** and **ADAPT** (Able Disabled All People Together), offer training in social integration and vocational skills for adults with mild autism. **Forum for Autism** also provides a network of resources, connecting parents to organizations that support developmental growth through group activities and workshops. In addition to these centers, social skills workshops are often very helpful for young adults, focusing on conversation skills, self-expression, and managing social interactions in a supportive group setting. These structured interactions can be key to building maturity and self-confidence.

If she has specific interests, vocational training programs that offer hands-on experience or internships can be valuable in exploring new strengths and areas of interest. If you’d like additional details on specific organizations or need help finding associations nearby, I’d be glad to assist further.

Hello Dr... I am 46 years old moderately built with a weight of 92 kgs, 5' 11", and a waist size of 40 inches (slightly obese). No Diabetes, BP, or any other lifestyle diseases. Slightly higher levels of LDL and good HDL. I have a history of CVT and I have been on warfarin for 12 years and my PT INR levels are between 1.8 to 2. I am a vegetarian with discipline in eating at the right intervals - 8 am, 1 pm, and 6:30 pm daily. I am moderately alcoholic once a month with a couple of beers or any hot drink with a limit of 2 drinks only during gatherings or parties. I am trying to lose weight and from last month, I started doing warmup exercises in the early morning for 45 mins along with Suryanamaskars and Pranayam (for which I had taken training during my childhood. In the evening I walk for 30-40 min 2.5-3.5 km. My problem is that after started working out since one month, I feel extremely sleepy between 1:30 to 2:30 pm (after lunch and take a nap of 15 min). And again I feel extremely sleepy between 5:30 to 6:30 which is forcing me to take a nap for 15-20 min. Both the times I get into sound sleep and snore as my wife complains. Night I go to bed at 10:30 and get up at early morning 4:45 but do not snore. Is this related to any underlying health issues that I need to get tested?
Ans: There are a few issues which can be noted:
1. Please increase your night sleep time to around 7 to 8 hours.
2. Consult a dietitian to work on reducing weight.
3. There should not be any reason that you have day time snoring but not in night time. It might be linked to day time sleeping posture. However, in general people who are obese more likely to snore. You can consult a sleep medicine specialist to get this worked out.
4. Eat fruits and vegetables in the diet. Vitamin D, B12 and E supplementation for shorter duration may help you in feeling revitalised, provided you may have deficiency.

Hi Komalji, I'm 61 years male with HbA1C- 9.36 FBS @180 and Random sugar level @ 265 calculated in recent lab tests . My LDL cholesterol (71) and BP is in control even though i am taking 10 mg of Rostarvasten tablet for cholesterol and Janumet 50/1000mg daily once after dinner and Diamicron XT 60mg once in a day before breakfast for sugar . Is this medication Ok or do I have to take Needle injected insulin to reduce my sugar level .If so how much quantity per day?
Ans: The acceptable HbA1c is less than 7% and fasting within 70 to 140 mg% and 2 hr post breakfast within 70 to 180 mg%. Your blood sugar levels are not in control and the medications you are taking are clearly insufficient. Either oral medications need to be increased or (and a better approach) would be to start on insulin.
There are many myths about insulin in people and they fear to get started on insulin. What is not understood that we need to have sugar under control and in good control to prevent the complications. Insulin which is administered is exactly what our body produce - an hormone and when body fails to produce, we need to give injectable insulin.

Please discuss with your doctor and ensure your blood sugar levels falls with in range. Starting on insulin and than adjusting oral antidiabetic medications is a good idea. Your LDL cholesterol is just close to the range and it ismost likely due to 10 mg of Rosuvastatin you are taking, so please continue that.

Best wishes,

Hi to the health expert(gynaecologist), I am a 24yr old lady working in an MNC with moderate work stress. I've pcos since i m 14 yrs old. I matured at the age of 11. I m a second daughter to my parents. During my teenage(5'4 height , weighed between 47-50kg , watery body with no muscle or bone mass), since i was too ambitious about getting a job and working hard in my studies, i didn't focus on eating well enough. I never skipped my meals or ate any kind of outside food(not even chips or biscuits). It's just that i am mostly stressed due to studies and eat only till my stomach says i m full. I didn't workout or ate enough protein till 22yrs old. Since last year , i m starting to look online and doctors suggestion regarding pcos and started taking care of my nutrition, lifestyle. Currently from the recent checkups I found out that have B12 and D3 deficiencies. Other than PCOS, my thyroid, insulin, prolactin, testosterone, female hormones, lipid profile etc.., everything is good. Currently, my height is 5'4, weight is 55kg, mostly water body with average muscle in legs atleast. I m focusing to take dry fruits , enough protein(eggs, dal, paneer, chicken) in my diet, along with working out atleast 3-4 times a week, sleeping peacefully. Getting periods between every 30-50 days. I m at the start of the journey to improve my health and nutritional deficiencies. I m not sure when I'll be married or gonna plan kids. Please advise me on how much time my body may take for regularising the periods since my body was irregular for 10yrs and how to understand if my health is good for child planning. Please advise me regarding any additional care required.
Ans: History noted. Taking into consideration your age of 24, with Pcos with consistently irregular periods, vitamin D and B12 deficiency, and stress factor as the prime motivator in your studies
Following advises: 1) You can continue taking low-dose ORAl contraceptive pills for a few months if you have irregular periods.
2) To prevent withdrawal bleeding, take progesterone-only pills if your periods last longer than six weeks. 3) To cure deficiencies in vitamins, B12 and D, take 60 k of vitamin D once a week for 12 weeks. For three months, both
4) PCO supplements that include metformin, melatonin, myoionositol, and chiroioinositol 5) Changes in lifestyle: 45-minute walk every day. Zumba, yoga, meditation, and more. 6) 7 to 8 hours of restful sleep 7) Diet: proteins (eggs, paneer, dal, and pulses) and green vegetables, Fruits with color, Dehydrated fruits. For omega-3 fatty acids, flax seeds
Taking fertility health into account tests for amh levels and specific hormones, such as TSH, prolactin, fasting insulin, and HbA1c, can be performed. To maintain your fertility, you can also proceed with freezing your eggs.

Resected Madam, I am a 72 years male . I had undergone left hemicolectomy with diversion ileostomy ( open "Surgery" )for carcinoma descending colon on 23 March,2024 and the stoma closure was done on 17th July,2024. As per the consultant Oncologist the carcinoma was localized , did not spread to other parts of the body and I was not advised to undergone chemotherapy etc for the same reason. Kindly advise which Yoga postures I can practice now to ease constipation and also the yoga postures I must not / avoid now. With Kind Regards,
Ans: After your surgery, gentle yoga postures can help ease constipation and improve digestion. Start with simple poses like Pawanmuktasana (Wind-Relieving Pose), which can relieve gas and promote bowel movements. Lie on your back, hug one knee to your chest, and gently press it down to your abdomen, then switch legs. Practicing Supta Baddha Konasana (Reclining Bound Angle Pose) can also be very calming and helps stimulate digestion. Breathe deeply and allow your body to relax fully.

However, avoid intense twisting poses (like Ardha Matsyendrasana) and deep forward bends as these may strain your abdominal area. Also, postpone advanced poses or any practice that puts pressure on your core until you’ve fully regained strength and mobility.

Consulting a certified yoga coach is essential to ensure you perform these poses safely, especially after surgery. A coach can help you adapt postures to your current needs and gradually increase the intensity as you progress.
