I've been a 35-year-old male diabetic for the last 2 years. I joined Sugarfit program for diabetes reversal at that time my weight was 73 kg and my height is about 6 feet. I had followed their medication and diet instructions which reduced my HBA1C from 8.5 to 7 but at the same time my weight was reduced to 63 in about 4-5 months initially they were saying it water loss and other things. As my weight is not increasing even after work out and protein intake. medicine precribedwere istamet 50/100- breakfast and dinner, oxramet10/500-lunch and I m on following sequencing salad-protien -fibre-carb-fire and grainless dinner, still my sugar is not under control and HBA1C is 8 in these month I m not sure whether they are treating my properly or not now my concern is on both weight gain and controlling blood glucose and sugarfit is not concern about my health not responding they do not have custumercare no . Please guide me what to do .
Ans: Pankaj Ji,
You are young and that age, aggressive and effective control of blood sugar is important. You are on nearly full dose of 3 anti-diabetic medications. and apparently doing a lot of physical activity, yet your blood sugar is not under control.

I would advise that please either switch to insulin or consult a physician to get medications restructured.

Regarding weight reduction, if it is fat loss, you need not to worry. However, if it is muscle loss, then, you probably need a little more attention. Some of the muscle strengthening exercises would help.

Hello Dr... I am 46 years old moderately built with a weight of 92 kgs, 5' 11", and a waist size of 40 inches (slightly obese). No Diabetes, BP, or any other lifestyle diseases. Slightly higher levels of LDL and good HDL. I have a history of CVT and I have been on warfarin for 12 years and my PT INR levels are between 1.8 to 2. I am a vegetarian with discipline in eating at the right intervals - 8 am, 1 pm, and 6:30 pm daily. I am moderately alcoholic once a month with a couple of beers or any hot drink with a limit of 2 drinks only during gatherings or parties. I am trying to lose weight and from last month, I started doing warmup exercises in the early morning for 45 mins along with Suryanamaskars and Pranayam (for which I had taken training during my childhood. In the evening I walk for 30-40 min 2.5-3.5 km. My problem is that after started working out since one month, I feel extremely sleepy between 1:30 to 2:30 pm (after lunch and take a nap of 15 min). And again I feel extremely sleepy between 5:30 to 6:30 which is forcing me to take a nap for 15-20 min. Both the times I get into sound sleep and snore as my wife complains. Night I go to bed at 10:30 and get up at early morning 4:45 but do not snore. Is this related to any underlying health issues that I need to get tested?
Ans: There are a few issues which can be noted:
1. Please increase your night sleep time to around 7 to 8 hours.
2. Consult a dietitian to work on reducing weight.
3. There should not be any reason that you have day time snoring but not in night time. It might be linked to day time sleeping posture. However, in general people who are obese more likely to snore. You can consult a sleep medicine specialist to get this worked out.
4. Eat fruits and vegetables in the diet. Vitamin D, B12 and E supplementation for shorter duration may help you in feeling revitalised, provided you may have deficiency.

Hi Komalji, I'm 61 years male with HbA1C- 9.36 FBS @180 and Random sugar level @ 265 calculated in recent lab tests . My LDL cholesterol (71) and BP is in control even though i am taking 10 mg of Rostarvasten tablet for cholesterol and Janumet 50/1000mg daily once after dinner and Diamicron XT 60mg once in a day before breakfast for sugar . Is this medication Ok or do I have to take Needle injected insulin to reduce my sugar level .If so how much quantity per day?
Ans: The acceptable HbA1c is less than 7% and fasting within 70 to 140 mg% and 2 hr post breakfast within 70 to 180 mg%. Your blood sugar levels are not in control and the medications you are taking are clearly insufficient. Either oral medications need to be increased or (and a better approach) would be to start on insulin.
There are many myths about insulin in people and they fear to get started on insulin. What is not understood that we need to have sugar under control and in good control to prevent the complications. Insulin which is administered is exactly what our body produce - an hormone and when body fails to produce, we need to give injectable insulin.

Please discuss with your doctor and ensure your blood sugar levels falls with in range. Starting on insulin and than adjusting oral antidiabetic medications is a good idea. Your LDL cholesterol is just close to the range and it ismost likely due to 10 mg of Rosuvastatin you are taking, so please continue that.
