Hi Sir will taking a remylin D helps in Hemoglobin? is there any relationship between hemoglobin and diabetes. What is the ideal hemoglobin percentage for men and women

Ans: Remylin D is a multivitamin. It helps in improving overall blood count by tackling the issue of small or large blood cells. However, it does not have Iron which is usually needed for improving Hameoglobin. However, please remember it has a role in iron deficiency anemia or when we have CBC which is deranged.
Please discuss with your doctor regarding specifics of why Remylin D has been prescribed.

Sir I have been asked to take Remylin D after dinner along with daily diabetic medicines? Is this enough or do you have take any other vitamins? My age 54 and diabetic for the past 7 years. My diabetes is under control

Ans: Would be good to check Vit D, Vit B12 levels and then the course of vitamin replacement and doses may be decided.

Good evening Doctor. I am 76 living with Diabetes and B.P for the last 7 years.i am on Tazloc AM 40 m.g in the morning and zitamet plus 500 m.g and Rozat 10 mg after dinner.I am continuing these drugs for a long time now.since i found vit.D level was mere 15.5 on many occasions when blood tests were taken,i am taking Remylin in the morning as per doctor's advice.My recent blood test reveals fasting sugar at 132 with h1abc at 6.2.My Tryclerizdes level is also marginally high at 155,but ldl and hdl levels are found within the range. I seek your valuable guidance in maintaining my overall health conditions and if any changes are needed, please let me know. I am on walking daily for not less than 45 minutes. I am also getting dizziness at times in the morning mostly and goes off after a certain time. Kindly advice me Doctor. Thank you

Ans: Dizziness in the morning could be due to postural hypotension
Whenever you get up from bed, turn to one side preferable to your right and get up and plabe feet on the ground by the side of the bed. Feel the Earth move your toes and gently get up stand with feet apart at shoulder width and gently move your toes and the start your day.
Your biochemical markers are OK. Not to worry.
But try and have your dinner by 6 pm. Even if you are on medication try and keep your medicine schedule between 7 am and 7 pm
So they are also well balanced
Avoid animal protein at dinner meal

Ans: Include iron rich foods along with vitamin rich foods
Iron rich foods
- Cereals- Bajra
- Pulses- Horsegram dal, masoor dal
- Fruit- watermelon, dates, Black raisins
-Green leafy veg - Moringa leaves, Colocacia leaves, Cauliflower leaves
- Nuts& seeds- Gardencress seeds, Gingelly seeds
-Egg, chicken,meat

Vitamin C rich food:
Orange, sweetlime, amla, lemon, tamrind pulp

Hi, I have diabetics from last 10 15 years. I live in USA. I tried every thing. I am vegetarian walk 7 to 8 mile regular walk. Eat Salad fruits and only two roti and dal every day . Taking medicine like Jardiance 10 mg , some insulin one time and control on food like namkin , sweets etc but Hemoglobin does not come down . It is 7.5 to 8.5 most of time. I don't understands why it is not coming down. Do you have any suggestion. Please advice.

Ans: Please let me know your age.
HbA1c 7.5 to 8.5% is not very bad. You can increase Jardiance to 25 mg. However, you are not taking Metformin which is a very good sensitizer and anti diabetic medicine, which need to be added to your treatment. If that is done, I beleive your blood sugar level will come within normal range.
Thereafter, continuing with healthy lifestyle as you have mentioned and some more uptitrating with insulin, your blood sugar levels may be within normal range.

Since your diabetes is for last 15 years, please get routine eye, kidney and metabolic health check up.

Hi Sir, My question is on tax in India on income earned in other country(we already paid tax in other country for that income). I stayed for 150 days in germany in deputation salary , and saved around 8L after paying tax in Germany. Do I need pay tax again in India on this income. How to show this income, while filing the returns in ITR, India.?

Ans: As your stay in India is more than 182 days; you are resident during the year. I understand that in earlier year also you were resident in India. Accordingly, your status is Resident and Ordinary Resident in India.

Accordingly, you are liable to pay tax in India for Income earned outside India also. However, you can take tax credit for taxes paid outside India.

In the Income tax return, you have to show income under salary head and required to show necessary details in Schedule FSI & TR.

Can i retire by 40, my age is 39. I have equity portfolio of 17 cr and monthly expense of 5 lacs...

Ans: Hello;

Yes you can surely retire now.

You may buy an immediate annuity for a sum of 15 Cr from a life insurance company.

It may yield you post tax monthly income of 5.25 L.

The balance 2 Cr you should invest in equity savings type mutual fund say for eg Kotak equity savings fund(moderate risk)as lumpsum for 10 years.

After 10 years it may grow into a corpus of around 5 Cr. If you use this to make a SWP(~ 3%) it may yield you a post-tax monthly income of 2.1 L, to beat inflation, in addition to 5.25 L per month as mentioned above. Returns from equity savings fund are assumed to be around 9% pa.

You may do lumpsum investment in SBI equity savings fund (moderately high risk).

The corpus will be around 12 L after 10 years assuming modest return of 9%.

You may begin a monthly sip of 50 K in a combination of pure equity mutual funds for 20 years.

You will reach your target of 5 Cr after 20 years assuming modest return of 12%.

Do supplement the monthly sip by investing part of annual bonus, if any, as lumpsum investments.

I am 33 investing 30K in MF 20K in PPF 10K in NPS 10K in VPF 10K in stocks all investment are monthly, by when I will be able to achieve 1Cr.

Ans: Hello;

With this judicious blend of monthly investments you may reach 1 Cr cumulative corpus in 7 to 8 years time frame.

You can't invest in PPF more then 1.5 L pa (12.5 K per month) cumulatively in all accounts under your name.

But if the additional 7.5 K per month is towards PPF account of your spouse or any other adult member in your family then it's okay.

Dear Sir, Request advise, I’m 32, earning 12L annually, with dependent parents (no income, ex-daily wage earners). I've an 8.5L home loan outstanding and expect 7L sum from insurance soon. Should I use this to pay down the home loan or invest in a SWP for my parents monthly income? Thank you.

Ans: Hello;

SWP on 7 L corpus wont be much therefore it is preferable that you utilise the insurance policy proceeds to prepay the home loan.

You may buy an immediate/deferred, annuity for a corpus of around 20 L, as pension for your parents, payable to you being nominee, after them.

Sir, I have did kidney and urine routine and values for BUN is low at 4.7 and urea at 10, uric acid at 2.80 all other parents are in range. Please advise if it's ok

Ans: As a doctor, we dont treat lab reports. These are always to be reviewed in context of presenting complaints.
Was it a routine test? why did you get this done? What are the other lab reports?

i have been administered glimster 1 and forxiga 10 is forxiga 10 lifelong pills

Ans: If these are the only medicine you are receiving for diabetes medicine, there is need for urgent review and discussion with your physician.

Metformin which is insulin sensitiser and first line drug for all Diabetes type 2 treatment guidelines (unless person has intolerance to it) is missing from your treatment.
Then, Glimestar is sulfonyleurea which may be stopped after a few years but Dapagliflozin (Brand name Forxiga) is useful drug and need to be stopped only in case of severe infections of UTI etc.

In short, get your treatment reviewed by a physician

Who exactly is he to tell you what to do and what not to do? He's doing this because you are allowing it, right?
If he already has begun to control certain aspects of your life and imagines that he is right, doesn't it look like a BIG RED FLAG?
Please think and act for yourself without being controlled by anyone. You are capable and you owe it yourself to steer your career your way. Emotionally blackmailing you by asking if your career is more important than the relationship: Say YES once and watch his reaction.
