I am senior citizen, have severe jaw pain, problem started after RCT in 2020. Consulted many dentists, few more rct redone, impacted tooth also removed. Treatment for trigeminal neuralgia going on for last 4 years, and recently treatment for oromandibular dystonia , medicines added. TMJ specialist treatment with splint and night guard also going on for lat 1 .5 years. Not much relief with left side jaw pain. Have shoulder and neck pain also. Please advise. Thanks,

Ans: Hello
It’s sad that you've been suffering for the past 4 years and no lasting solution has been provided. It is quite possible that your problem is not dental in origin. You have clearly stated that you have consulted several dentists (including a TMJ specialist) so it is safe to assume that you no longer have any untreated dental infection or abnormality.
My advice to you would be to consult a reputed Neurologist or Pain Management specialist.

Dear Dorctor, I did RCT for my (right upper) 6th around 3 years ago & 7th around 1 year ago my 8th tooth removed over 25 yrs ago & now I am 65. Though 6th is ok but 7th some times paining. Do I need to go for RCT again ? Kindly respond. Thank u

Ans: Hello
Your dentist will most likely take xrays to check if one of the mentioned teeth is reinfected. In which case retreatment may be necessary. However please note that upper molar teeth can sometimes show misleading signs of tooth infection if sinuses are inflamed. This happens because the roots of the upper molars are in close contact with the sinuses. So your dentist will try to rule out that angle, too.
The exact cause of your occasional pain can only be determined after careful clinical examination and correlating the findings with xrays and your history.

I am 50 male. Toothache, sometimes appears and disappears in Right upper and lower jaw. Pain in chewing. Also Cold/ hot sensation.

Please consult your dentist soon rather than suffer. A clinical examination will usually reveal which tooth/teeth are affected. It's very likely you have deep untreated decay somewhere. From your symptoms it appears that there's a low grade infection somewhere. Since you cannot precisely pinpoint which tooth is paining a full jaw Xray (called an OPG) may also be necessary in order to reach a proper diagnosis.
Once the affected tooth/teeth are identified and the appropriate treatment done you should be able to eat and drink comfortably.

Ans: If you indeed have migraine and if you are getting more than 6 attacks a month or if the other drugs used for the prevention of migraine are not effective in your case, you can try drugs known as CGRP inhibitors after consulting a neurologist or an internist

Hi, i have dental issues , my upper left side of the jaw has teeth no 4&5 on which RCT is in progress since march 2023. it was very painful for tooth4 and mild for 5, so i changed dentist on 1 may and new dentist took full month of may to do RCT for 4 & 5 again. now pain has disappeared in 5 but remains in 4. the new dentist says that there is a pocket between 4 &3 and i have copy of xray that shows that. i m not sure this dentist is telling the right reasons, as some other dentist says that condition of 4 is so bad that filling inside it is visible from outside and extraction is last resort.. my question is what should i do? protecting a tooth at all costs is more important or shall i go for extraction.

Sorry to hear that even after two months your RCTs have not been completed to your satisfaction. There could be a variety of reasons why this is happening. Despite the impressive advances in Dental treatment procedures we still see some cases that are untreatable. And while we spare no effort to save natural teeth, in some cases extraction becomes unavoidable. What is important is that you get complete relief from pain/discomfort. Whether to extract it or continue with the treatment is a call you have to take in consultation with your dentist.
