HI ,i am 44 years old, i practice yoga for 1 hour every day and go on hikes once in 2 months. Have a meniscus tear on right knee grade 3 . of late i get issues such as muscle pull or tear pretty often. The osteopathy doctor i visited recently said that my muscles are weak. i visited the doctor for what was diagnosed by a physiotherapist as Muscle impingement as i am unable to lift my hand sideways to my shoulder level. While visiting the osteopath he pressed back of my neck at C2 and C3 area and the pain was gone and could lift my hand normally. I am unsure as to what i need to do. Please advise.

If it is impingement Physiotherapy plays a crucial role in the management of shoulder impingement by focusing on reducing pain, improving shoulder mobility, and strengthening the muscles around the shoulder joint. You can also ask Physiotherapist to use Ultra sound, Laser and TENS for alleviating pain.

I had frozen shoulder problem as told to me in my left hand and from November, 2020 to August, 2021 i consulted various reputed orthopedic surgeons and even got treatment from Safdarjang Sports Injury Centre New Delhi and as per advice got physiotherapy treatment for full one year but to no avail. Then in September, 2020 i consulted one of the top most Ortho Surgeons who after xray etc. told me that he will give me two injections(whereas he gave three such injections) and my problem will be got with physio treatment in three months but from day one his physio told him that my shoulder is very stiff and only solution is to get MRI done and then operation be done which was done in October, 2020 and I was told that after five week physio i will be ok but even after operation there was so much pain at the time of physio and afterwards that i could not have sleep for 20/22 hours and then Doctor extended time limit to two months and then to three months but even after five months there was no relief. ultimate i stopped treatment and consulted another ortho in south delhi reputed hospital he told me that my veins were weak and at the first place i stopped hard core physio and that surgery was not a best option. Though after I stopped physio and started doing light exercises at home there is slight improvement but at times i have great pain at lower of my shoulder/shoulder and upper half portion of left hand. It is one year since i have started treatment from present ortho. What to do?

i am 55 years old. Retired defense pilot. Have lots of sports injury which is showing up at this age. Body biochemistry is all with in limits, only fasting sugar is 108. For last few months i am having pain in outer left thigh muscles which increases during sleep as well as while lying down resulting in sleepless nights hence general body fatigue due lack of sleep. My exercise includes 5-6 km walk in the morning and occasionally thrice a week cycling. Orthopedic , General surgeon, neurologist as well as ayurvedic doctor has been consulted with MRI report. FINDINGS: Dorsal and lumbar vertebrae are normal in attenuation and alignment. Straightening of lumbar lordosis. Desiccation of lumbar intervertebral disc from L2-L3 to L5-S1 levels. Mild diffuse disc bulges at L2-L3, L3-L4 and L4-L5 intervertebral discs indenting anterior thecal sac with mild narrowing of lateral recesses and neural foramina. No significant compression of the cauda equina. Mild disc bulge at L5-S1 level indenting anterior thecal sac. Bilateral facet arthropathy is seen at L4-L5 level. Dorsal spinal cord, conus medullaris and cauda equine are normal. Para spinal soft tissues are normal. IMPRESSION: Mild lumbar spondylosis with mild diffuse disc bulges from L2-L3 to L5-S1 levels. Kindly help me and advice
Ans: Hello Partha,
You need to consult a physiotherapist who knows the Mckenzie treatment for slipped disc. At times pain in outer thigh can be due to slipped disc.
Or you may have a hip bursitis which will not come up in lumbar spine MRI, only hip MRI will show it. Outer thigh pain while sleeping on left side can cause pain if it is a left hip bursitis.
Or this could be a Iliotibial band tightness which is a band of muscles on the outer thigh.
A good sports physiotherapist who also knows the Mckenzie treatment needs to evaluate you, understand the root cause and accordingly give you the treatment and you should be fine.
