Any habit happens because of repetition...similarly to form a new useful habit, you need to repeat it sufficient number of times for it to become an automatic behaviour.
But you will not be able to form that new habit till you can convince your brain that what you lose from the old habit, you will somehow gain with a new useful habit.
So, ask yourself, what it is that I gain by being on my phone late night? Is it fun? Is it keeping me away from loneliness?
Now create, the same through your new habit...the same fun, the same company...you can join a gym, you can be a hobby...
These are mere examples...now apply it on to your situation and change...
